This spreadsheet uses the standardised DASS test to capture a metric of depression, anxiety and stress with a set of x questions. The spreadsheet has been set up so that you can easily ask and capture the questions via a mobile interface, and has initial, 3-week and 6-week scores, so that you can compare metrics over time for your clients.
